q       Media Release Form (Signed by Parent)

q       Player Contract (Signed by Player & Parent)

q       Player Profile Worksheet (New Players Only)

OR

q       Player Profile Updates (Returning Players)

q       Membership Form (Leave Team Info Blank)

q       Medical Release Form (Signed by Parent & Notarized)

q       Player Proof of Age if not on file with Soccer Forces

    (copy of Birth Certificate or Driver’s License)

To be added to roster for 2007/08 Soccer Year deliver all documents (completed) along with $20 state registration fee by July 16, 2007 to:
